Orthodontic treatment is designed to be safe and predictable, but from time to time unexpected issues can happen. A loose bracket, poking wire, or lost aligner can feel stressful, especially if it happens outside normal office hours. Knowing how to handle orthodontic emergencies can help you stay calm, avoid pain, and protect your treatment progress.

At New Braunfels Braces, we want every patient to feel prepared and confident, even when something goes wrong. This guide explains the most common orthodontic emergencies and what you can do until you are able to see your orthodontist.

What Is an Orthodontic Emergency

An orthodontic emergency is any situation involving your braces or aligners that causes pain, discomfort, or damage to your appliance. While most issues are not dangerous, they can interfere with treatment if not addressed properly.

True emergencies involving severe pain, swelling, bleeding, or trauma to the mouth should be treated immediately by a medical professional or emergency dentist. Most orthodontic problems, however, can be managed safely at home until your next appointment.

Broken or Loose Brackets

A loose or broken bracket is one of the most common orthodontic issues. This often happens from eating hard or sticky foods or from accidental impact.

If a bracket is loose but still attached to the wire, leave it in place and avoid touching it. If it has completely come off, save it and bring it to your next appointment. Do not try to glue it back on yourself.

In the meantime, avoid hard foods and chew on the opposite side of your mouth if possible.

Poking or Irritating Wires

Wires can sometimes shift and begin poking the inside of your cheeks or lips. This can be uncomfortable and even painful.

You can use orthodontic wax to cover the sharp end of the wire. If the wire is long, you may be able to gently push it back into place using a clean cotton swab or the eraser end of a pencil.

If the wire cannot be adjusted and continues to cause irritation, contact your orthodontist for an appointment as soon as possible.

Lost or Broken Aligners

If you wear clear aligners and lose or break one, contact your orthodontist right away. In many cases, you may be instructed to move on to the next set of aligners or temporarily wear the previous set.

Do not skip multiple aligners or stop treatment without guidance, as this can affect your progress and results.

Loose Bands or Spacers

Bands and spacers help guide tooth movement and maintain proper spacing. If one becomes loose or falls out, contact your orthodontist for advice.

Do not try to replace bands or spacers on your own. These components must be positioned correctly to avoid disrupting treatment.

Mouth Sores and Irritation

It is common to experience mouth sores or irritation when starting braces or after adjustments. The lips and cheeks need time to adjust to the new appliances.

Using orthodontic wax on brackets that rub can reduce friction. Rinsing with warm salt water can also soothe sore areas and promote healing.

Over-the-counter pain relievers may help, but discomfort usually improves within a few days.

Trauma or Injury to the Mouth

If you experience an injury to the face, mouth, or teeth while wearing braces, seek medical attention immediately. Even if the braces appear intact, underlying damage may not be visible.

After receiving medical care, schedule an appointment with your orthodontist to evaluate your appliances and ensure treatment can continue safely.

When to Call Your Orthodontist

You should contact your orthodontist if you experience persistent pain, significant appliance damage, or any issue that interferes with eating, speaking, or sleeping.

Even if the problem seems minor, it is always better to ask. Early intervention prevents small issues from becoming larger ones.

What Is Not an Emergency

Some situations may feel urgent but are not true emergencies. These include mild discomfort after adjustments, food stuck between brackets, or a slightly loose elastic band.

These issues can usually wait until your next scheduled appointment.

Preventing Orthodontic Emergencies

Many orthodontic emergencies can be avoided with proper care.

Avoid hard, sticky, or crunchy foods that can damage braces. Wear a mouthguard during sports or physical activities. Follow your orthodontist’s instructions carefully and attend regular appointments.

Taking care of your appliances reduces the risk of unexpected problems and keeps your treatment on track.
